Your First Appointment

Please arrive 15 minutes before your scheduled appointment time in order to fill out necessary informational forms. If your visit is to be covered by insurance, we will need to see your card(s). Worker's compensation claims also require authorization from your employer but not for the first free visit. Motor vehicle claims require insurance carrier name and claim number.

Please bring these items to the center on your first visit.

• Current insurance information and cards

• Mention you found us on the web site and Dr Stoxen will consult with you at no charge for the first visit. 

• Please briong any list of medications you are currently taking

• Please bring any resent medical records MRI/CT scans and/or x-rays form other physicians pertaining to the problem you want treated.

• Please bring your insurance card and your spouses insurance card so we can verify your benefits and give you a quote on services right on the spot.

If you were involved in an auto accident please bring these additional items with you.

• Please bring the above items including your insurance card, spouses insurance card and your auto insurance card. (We can bill your insurance for care resulting from an auto accident. Your heath insurance company can get reimbursed from the auto insurance carrier responsible for the payment)

• The other parties auto insurance information

• The police report

• Your attorneys information (if you have one)

• Hospital, emergency room records, x-rays, and results of diagnostic tests taken at a facility relating to the injuries sustained in the auto accident.

If you were injured at work and want the treatments covered under worker's compensation, we can do that if you bring these items.

•  Please bring the above items including your insurance card, spouses insurance card and your companies work comp insurance carrier name and the adjuster handling your claim

•  Your attorneys information (if you have one)

•  Hospital, emergency room records, x-rays, and results of diagnostic tests taken at a facility relating to the injuries sustained in the auto accident.

No Insurance - The free visit coupon pay for your entire visit up to the examination, 2 x-rays and report of findings.

Medicare - we do not participate with Medicare

Medicaid - we do not participate with Medicaid
